If you would like to extend your coverage beyond the aggregate limit that is provided through the Student Accident & Sickness Plan, you may enroll in the Major Medical (Buy-up) Plan. The Major Medical Plan provides benefits only after the Student Accident & Sickness Plan Aggregate Limit has been exhausted. Coverage is then provided for covered expenses at 80% of the Usual, Reasonable & Customary (URC) charge. Please click on the button below for information on the Major Medical (Buy-Up) Plan.
To enroll, you must download the enrollment form, complete it and submit it along with your payment to EIIA before October 15, 2008.
